How It Works
1. Fill Out the Google Form & Pay
Note what skills your student has learned in each subject listed & then upload samples in each category. The form will walk you through everything including payment.
Timeline: Submit between May 1-July 8, so you can turn in your letter by August 1.
2. Evaluation Review
I will let you know that I have received everything and begin reviewing your portfolio, looking for growth over the school year — not perfection.
3. Receive Your Letter
You’ll receive a signed evaluation letter via email, confirming adequate academic progress, ready to submit to your school division.

Note:
- Submit 1 form per student
- It is helpful to read over the form before beginning, as many questions are answered once you see what is asked.
- If you would like a meeting, please complete the form and payment first; You will see the calendar link at the end of the form.
- I take 10 submissions per month & will close the form when the month is filled.

Frequently Asked Questions
It must illustrate “adequate academic progress”, which means that your child has shown growth over the school year.
I am looking for:
- Improvement in skills (especially reading, writing, and math)
- Consistent instruction
- Evidence of learning over time
Your child is not being compared to public school standards or other students.

No. The meeting is completely optional.
Many families choose to skip the meeting and simply submit their portfolio.
The optional meeting is helpful if you would like to:
- Explain your child’s progress
- Share details about projects
- Discuss learning differences or gaps
Please submit a minimum of 3 and maximum of 10 samples per subject. The goal is to submit what helps you show your student has learned the skills you list in the form.
Helpful items to include:
- Beginning-of-year & End-of-year samples on the same topic
- Assessments, projects, or assignments that show multiple skills learned.

You must include:
- Language Arts: which is broken into reading comprehension, writing, grammar, & spelling
- Phonics: for K-2nd grade students, or students working on these skills
- Math
Other subjects (science, history, electives) are optional but helpful.

Clear photos taken with your phone are perfectly acceptable.
That is absolutely okay.
I have experience working with a variety of learning differences.
Your child will be evaluated based on their individual growth and progress, not a standard comparison.
I specialize in:
- Kindergarten through 6th grade
This is dependent on how many submissions I receive.
You will get your letter by July 15 at the absolute latest; however, my goal is much sooner.
I will be in contact and update you as soon as:
- Your portfolio is submitted, including payment.
- (And meeting is completed, if applicable)
You will email the completed letter to your local school division superintendent by August 1.
